This form is used to request a social security and/or social security disability benefit statement. Visit the Social Security site for information on how to request a social security benefit statement (Form SSA-1099/1042) online, over the phone, or in person, or print a payment detail statement from your personal "my Social Security" account. - Form 4506-T
This form is used to request a tax return transcript. To request the IRS Tax Return by phone call 1-800-908-9946 or complete Form 4506-T on line at the IRS website. - Child Support
To request Child Support Payment History Statement visit the New York Child Support website or call the helpline at 1-888-208-4485. - EOP Recommendation Form
This form should be used by first-time applicants only and given to high school counselor, teacher, minister, rabbi, priest, lawyer, social worker, supervisor, or manager who will write in support of your application.
All Fall 2024 applicants must submit proof of taxable and non-taxable income from the 2022 calendar year to determine financial eligibility for EOP. Application is now open.
Taxable Income Sources: Federal Tax Return 1040, or 1040A, or 1040EZ along with W-2s and schedules; and NY State Tax Return IT 201.
Non-Taxable Sources: Social Security, Social Services, Child Support, Veteran's Benefits, or Workers Compensation.
